Objective of the Shoulder Ultrasound Scan

The shoulder scan aims to provide a detailed assessment of the significant musculoskeletal structures in the shoulder using ultrasound. The shoulder is one of the most used and injured joints. It comprises a number of tissues including muscles, tendons, ligaments, bones and cartilage. The shoulder scan uses ultrasound waves to see inside the shoulder and look at these structures. It is a quick and painless test that is completely safe. The results of the shoulder scan are discussed with you following the scan.

The shoulder ultrasound scan can assess:

What is the Shoulder Ultrasound Scan

A shoulder ultrasound scan is a diagnostic imaging test that uses high-frequency sound waves to create images of the structures inside the shoulder joint. It can be used to evaluate structures within the shoulder such as tendons, muscles, shoulder joints and other soft tissue structures.

The rotator cuff tendons: hold the shoulder together

The shoulder joint is held together by the rotator cuff.  The rotator cuff is a group of four muscles and tendons that connect the shoulder blade (scapula) to the upper arm bone (humerus). These muscles and tendons work together to control and stabilize the shoulder joint, allowing for a wide range of motion and strength. The four muscles that make up the rotator cuff are the supraspinatus, infraspinatus, teres minor, and subscapularis. The tendons of these muscles attach the muscles to the humerus and help to hold the head of the humerus in the shallow socket of the shoulder blade. Rotator cuff injuries, such as tears or inflammation, are a common cause of shoulder pain and can limit the range of motion and strength in the shoulder.
Injuries involving the shoulder are very common in adults, particularly with an injury, repetitive strain and over-use. These injuries can be short-term (acute) or long-term (Chronic). Acute injuries could be caused by sudden traumatic events such as falls or accidents. Chronic injuries are usually due to repetitive microtrauma to the area, however, in some instances may also be a consequence of an acute injury.

The cause of your shoulder pain is however extremely wide and there could be one or many possible causes, some more serious than others. Benefits of having the Shoulder Ultrasound Scan

The test is painless, safe, and can be done quickly. Your doctor may require a shoulder ultrasound scan along with other imaging tests such as an X-ray or MRI to plan the best treatment. Ultrasonography is a noninvasive imaging modality and is widely used for the assessment of the shoulder including the rotator cuff. It can provide clinically useful information on a wide range of conditions affecting the shoulders including the tendons, ligaments, synovial space, and surrounding soft tissues.

What is a Shoulder Ultrasound Scan?

A shoulder ultrasound is a procedure that uses ultrasound imaging to evaluate various parts of the shoulder, like the rotator cuff and the surrounding soft tissues.

The rotator cuff consists of a group of muscles and tendons that stabilise the shoulder joint. The shoulder ultrasound scan is the most requested musculoskeletal ultrasound test and provides a dynamic assessment of the shoulder.

Ultrasound scans are extremely safe with no known risks. Unlike X-rays, the patient is not exposed to ionising radiation. The exam is also quick and easy, making it appealing to patients who don’t have much time to spend in the waiting or exam room. There is no specific preparation required for shoulder ultrasound scans. You can eat or drink as desired before and after the procedure.

However, you might be requested to bring the doctor’s request form and any relevant previous imaging when coming for the scan.

Why You Need a Shoulder Ultrasound Scan?

There are many reasons for a shoulder ultrasound scan. The most common reasons for the scan are;

  • Sub-acromial shoulder bursitis is a thin fluid-filled sac containing fluid within the shoulder joint. It ensures smooth movement when lifting the arm. The inflammation of this sac is called sub-acromial bursitis and can result in shoulder pain when you try to lift your arm above the head level.
  • Rotator cuff tear – Another reason to get a shoulder ultrasound is to check for rotator cuff pain or tear. The rotator cuff can suffer from various injuries, such as tendon inflammation and partial and full-thickness tears. An ultrasound is an excellent tool for assessing rotator cuff tendon problems and subacromial bursitis. In many cases, both can be seen together.
  • Sub-acromial shoulder impingement – Shoulder impingement is a term used to describe a specific type of shoulder pain which can be the result of a number of conditions. Other conditions like long head of biceps, tendon pain and shoulder calcific tendonitis are also included. Dynamic assessment can be performed using ultrasound to look at the movement of the tendons and bursa while lifting the arm. The ultrasound can visualise the pinching of the bursa during shoulder movement, confirming the diagnosis of shoulder impingement.
  • Shoulder calcific tendonitis – This condition can lead to significant shoulder pain without any specific reason. The shoulder movements can be significantly restricted. Ultrasound is perfect for assessing the presence of calcification and inflammation and to rule out other causes of shoulder pain like rotator cuff tear.

A shoulder ultrasound can also diagnose frozen shoulders and acromioclavicular joint arthritis. The scan is also used to treat shoulder pain through ultrasound-guided injections like corticosteroid injections, among other forms of treatment.

Three Main Causes of Shoulder Pain

There are many possible causes of shoulder pain, aches and stiffness. Some causes of the pain, like muscle sprains, are relatively safe and usually recover naturally within a week with ample treatment such as rest and plasters.

Shoulder pain is mainly caused by injuries associated with lifting heavy items like weights and is quite common among professional athletes.
